An overview of systematic reviews on efficacy of nonpharmacological management of preoperative anxiety in adult patients undergoing elective surgery

Objective To overview the systematic reviews on efficacy of non-pharmacological management of preoperative anxiety in adult patients undergoing elective surgery. Methods In August 2021,CNKI,VIP Full-text database,Wanfang Full-text Database,China Biomedical Literature Database and Joanna Briggs Institute,the Cochrane Library,PubMed,Embase,CINAHL,and psycINFO databases were searched to collect systematic reviews or meta-analysis on the efficacy of non-pharmacological management of preoperative anxiety in adult patients undergoing elective surgery. 2 researchers screened systematic reviews and extracted the data independently. AMSTAR 2 scale and GRADE system were used to assess the quality of included systematic reviews. Results A total of 19 systematic reviews were included. The evaluation results showed that the methodological quality of 2 pieces of the literature was low and the methodological quality of 17 was critically low. Evidence for 9 non-pharmaceutical management interventions was evaluated. The results showed that optimization of preoperative management and application of non-pharmaceutical techniques had positive effects on preventing or alleviating preoperative anxiety. However,the grade of evidence quality is not high,and only a piece of evidence is intermediate. Conclusion Non-pharmacological management may relieve preoperative anxiety relatively. However,the methodological quality and evidence quality of the study are low,so it is necessary to use evidence carefully and carry out more standardized and rigorous studies.

Development of evidence-based popular works of swallowing safety for home-dwelling disabled elderly

Objective To design and make popular works of swallowing safety for the home-dwelling disabled elderly. Methods The evidence-based nursing and Delphi method were used to form the best evidence summary through literature retrieval and appraisal, evidence extraction and evidence level evaluation. The swallowing safety education system was constructed through 2 rounds of expert consultations. Based on this system, the content was refined and transformed into popular works, and then the popular works were designed and developed by multidisciplinary teams. Results 31 pieces of evidence were summarized from 8 aspects, such as safety education objective,identification of dysphagia,swallowing function exercise,oral management,etc. 2 rounds of consultations showed good initiative,authority and coordination,and the final system included 4 primary indicators,11 secondary indicators and 21 tertiary indicators. The popular works of swallowing safety included manuals,videos and audios,which were easy to understand,vivid and point-prominent. Conclusion The development method of popular works of swallowing safety for home-dwelling disabled elderly is scientific and feasible. It is suggested that creators should strengthen the scientificity,people-orientation and originality of popular works. This study can provide methodological references for other scholars to develop health popular works.

Establishment and practice of smart aged care platform based on hybrid aware model

Objective To design,establish and practice the smart aged care platform,to ensure the safety of community-home-based older adults,to improve the quality and comfort of aged care. Methods The hybrid aware model was used to build the concept framework of smart aged care platform,and the smart aged care platform was practiced in Chongqing community-home-based elderly health service technology promotion base. Results The smart aged care platform consists 5 subsystems,namely physiological data sensing system,environment data sensing system,location data sensing system,activities data sensing system and decision support system,respectively. It was divided into 4 system architectures,namely data acquisition layer,data interaction layer,business performance layer and application management layer. Until June 2020,the platform has been in use for 7 months,and there are 180 signed old people. The incidence of adverse events is 0,and the satisfaction rate of users and their families is 96%. Elderly users believe that smart aged care platform can enhance psychological security,meet the needs of medical care,and maintain the autonomy of life,but also report the personal privacy concerns and technical discomfort. Conclusion Although there is still space for improvement,the smart aged care platform based on hybrid aware model runs well with positive user evaluation,which is expected to provide references for the intelligent transformation and upgrading of the elderly health service industry.

Establishment and clinical practice of a bleeding risk grading management scheme for catheter contact thrombolysis

Objective To explore clinical effects of implementing a grading management program of bleeding risks in patients with deep venous thrombosis(DVT) of lower limbs performed by catheter directed thrombosis(CDT). Methods The research classifies bleeding risk levels and formulates management programs based on the value changes of plasma fibrinogen and plasma D-dimer during CDT. 90 patients performed by CDT from January 2018 to December 2019 were selected and randomly divided into an experimental group for a grading management program and a control group for a conventional nursing program with 45 cases in each group. The safety,effectiveness and hospitalization satisfaction of CDT therapy in 2 groups were compared. Results The proportion of bleeding events in the experimental group was different from that in the control group(P<0.05);the effects of venous patency and thigh swelling in the experimental group were improved compared with those in the control group(P<0.05);the satisfaction of nursing service and clinical efficacy in the experimental group was higher than that in the control group(P<0.05). Conclusion The bleeding risk grading management program of CDT can reduce the occurrence of bleeding events and further improve the clinical efficacy and patients’ satisfaction.
